SUMMARY OF BENEFITS
FOR THE PLAN YEAR BEGINNING DECEMBER 1, 2007

*MEDICAL INSURANCE:

The district offers a self-funded medical insurance plan.  Claims are paid through AAG, our third party administrator.  The district has selected Healthsmart as its preferred provider network.  By using this network the employee is able to receive a discount on medical services received.  Please contact the Employee Benefits Department at 580-5030 for information on rates, coverage, and enrollment.

*DENTAL INSURANCE: 

La Joya I.S.D. offers two dental options under its self-funded dental plan. Please contact the Employee Benefits Department at 580-5030 for information on rates, coverage and enrollment.

*VISION INSURANCE:

Avesis offers an economical way to purchase eye examinations, eyeglasses and contact lenses from member doctors.  At the time of your visit, you generally only pay a co-payment for your examination.  Materials require additional co-pay.  Additional services requested may result in additional charges.    Please contact the Avesis representative for rates and enrollment.

*CANCER INSURANCE:

Cancer insurance is a supplemental plan that provides direct payments to the insured to assist in offsetting expenses incurred in the treatment of cancer.  See an enroller for rates, coverage’s, and enrollment.

*HEART & STROKE INSURANCE:

This is a supplemental insurance plan that provides direct payments to the insured to assist in offsetting expenses incurred in the treatment of these illnesses.  See an enroller for rates and enrollment.

*LIFE INSURANCE:

The district provides each eligible employee with basic life insurance coverage in the following amounts:
                        Professional Employees          $20,000.00
                        All others                                $15,000.00

Additional coverage is available on an optional basis.  See an enroller for rates and enrollment.

*DISABILITY INSURANCE:

Disability insurance provides income replacement benefits in the event you are unable to work due to an accident or illness.  Employees can choose from a selection of features they feel best meets their financial needs.  The coverage selected must not exceed 66.67% of your salary.  The cost depends on various factors.  Pre-existing condition limitations may apply.  Please see an enroller for rates and enrollment.

*SECTION 125 “CAFETERIA” PLAN:

The “Cafeteria” plan allows for tax savings when qualified insurance premiums are deducted from gross salary.  Reducing gross salary reduces the amount of earnings on which federal income taxes are calculated and deducted.   As a result, the amount of taxes that the employee is required to have withheld from their paycheck is reduced.  Certain restrictions involving changing benefits during the plan year may apply.
